The CPRC (community support) Supervisor is responsible for the supervisory oversight, training, documentation, and coordination of community support and wrap around support services for adults with severe and persistent mental illness as well as supervising other applicable staff. Responsible for conducting team meetings and individual supervision that promotes client and staff growth. Participates in all aspects of continuous quality improvement. Creates a positive atmosphere within the team. Exercises sound judgment and initiative in ensuring that the integrity of services are consistent with Adapt’s mission, philosophy, and service principles. The CPRC Supervisor is required to share administrative on-call to assure that individuals served receive adequate clinical and support services to live successfully in the community.

This is a full-time, benefit eligible position.

ESSENTIAL FUNCTIONS – JOB SPECIFIC

• Has a working knowledge of CPRC and Community Support Standards as outlined in the DMH Core Rules and assures staff follows policies and procedures
• Responsible for ensuring employees understand and abide by the Corporate Compliance plan and applicable regulations and standards for complying with the plan
• Ensures that program meets licensing and certification requirements by working with Clinical Director and staff on preparation for licensing and certification visits as well as actively participating in licensing and certification visits
• Assures that transportation services are safe and reliable by ensuring staff follow transportation manual and having required safety equipment in their automobile
• Ensures the completion of clinical assessments and related paperwork according to established timelines
• Ensures quarterly reviews of recovery plan are completed in a timely manner
• Works well with clients as well as others involved in client care
• Establishes rapport and a therapeutic relationship
• Shows clients respect and compassion
• Assures communication and cooperation with affiliates (FCM, Probation/Parole, Regional Center, SCLP), landlords and other agencies to ensure continuity of care
• Assures clients maintain community funding by staying up to date on Medicaid re-certifications, ensuring SCLP paperwork is thorough and complete and ensuring staff are assisting clients in applying for SSI/SSDI and Section 8 housing
• Maintains oversight of clinical records by assuring documents meet CPRC regulations and Medicaid billing guidelines, recovery plan, progress notes and quarterly review paperwork are completed in a timely manner and are neat and thorough, incident and hospitalization reports are completed in a timely manner, and discharges are completed in a timely manner
• Participates in Continuous Quality Improvement (CQI) activities as assigned by completing all QC items monthly and submitting by the 5th of the following month
• Participates in the development of and implementation of annual strategic plan and CQI plan
• Exhibits cost consciousness by monitoring billing cycles and revenue budgets to stay within targeted goals,
tracking program census and aggressively monitoring client utilization of services
• Completes all tasks related to the hiring of new staff, assures personnel files are complete and up to date, assures payroll is completed and sent by deadline
• Assures annual employee evaluations are done in a timely manner
• Provides staff with direction, guidance, support and clarity of roles and definition
• Facilitates and promotes teamwork with the team and company by promoting a proactive rather than reactive atmosphere, bringing enthusiasm and a positive attitude to the job and maintaining a high level of staff energy
• Demonstrates the knowledge of legal issues and civil rights relevant to work setting
• Conducts activities in a culturally competent manner
• Keeps supervisor informed by meeting with supervisor for individual supervision as scheduled and updates supervisor on possible discharges or other pertinent client issues
• Is open to constructive criticism and makes apparent efforts in areas of self-improvement
• Attends in-service training given and participates in meetings held by the company
• Attends outside trainings to address areas of need
• Maintains own certification in CPR, First Aid and CPI
• Demonstrates knowledge of psychotropic medications
• Promotes holistic, person-centered care and ensures staff are providing services that focus on overall health and wellness of clients
• Presents a positive and professional image of Adapt and conforms to appropriate work behavior expectation to include dress, punctuality and professionalism
• Demonstrates efficiency, flexibility and attention to detail
• Assumes any other responsibilities as directed
• Works with rare absences or tardiness
