The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center.
PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.

Primary Purpose
Provides primary medical care for Senior Care Partners P.A.C.E.’s participants under direct supervision of a licensed physician and according to Senior Care Partners P.A.C.E. policy.
Key Responsibilities
Performs comprehensive history and physicals for new referrals.
Performs interval history and physicals on established participants, completed quarterly or as indicated.
Evaluates and treats participants with episodic illnesses.
Assists the IDT in formulating a plan for the coordination of 24-hour care.
Participates as a member of the IDT and takes part in participant care planning.
Documents participant’s changes appropriately in the medical record and communicates participant changes to the Physician and other team members.
Monitors the participant’s quality of care through collaboration with health care professionals and acts as their resource.
Participates in the management of the participant’s care by writing orders for appropriate medical care and determining need for consultation to ensure the continuity and quality of care.
Monitors supplies needed and communicates with appropriate staff to restock.
Works under the direct supervision of the Medical Director/Primary Care Physician (PCP) as described in the Nurse Practitioner Collaborative Practice Agreement.
Must care, support, and collaborate with everyone involved in the participant's care including the participant’s family using the patient-family-centered care approach.
Assists in all Senior Care Partners P.A.C.E. clinics as needed.
Performs other duties as assigned.
Leadership Responsibilities
This position has no professional leadership responsibilities.
Qualifications
Must have a Master’s in Science of Nursing/Nurse Practitioner degree.
Must be a graduate of an accredited Nurse Practitioner educational program.
Must be licensed as a Registered Nurse in the State of Michigan.
Must be licensed as a Nurse Practitioner in the State of Michigan.
Minimum of one year of experience with frail elderly population.
Strong communication skills, both written and verbal.
Well-developed ability to show discretion and maintain confidentiality.
Proficiency with MS Office Suite, Word, Outlook, system applications, etc.
Specific Requirements
Demonstrates our core values of compassion, integrity, teamwork, and excellence.
Possess current CPR certification by AHA or willing and able to obtain as offered by Senior Care Partners P.A.C.E.
Only act within the scope of his or her authority to practice.
Must be knowledgeable of current medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
Must be willing to seek out new methods and principles and be willing to incorporate them into existing medical practices.
Must possess the ability to make independent decisions when circumstances warrant such action.
Must possess the ability to communicate tactfully with personnel, participants, family members, visitors, government agencies/personnel, and the public.
Must have proven leadership experience and skills with the ability to get along harmoniously with others in a team environment.
Must possess the ability to plan, organize, develop, implement, and interpret Senior Care Partners P.A.C.E. goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to manage difficult participants.
Acceptance of Senior Care Partners P.A.C.E. philosophy.
Work Environment
Has a visible presence while working in the center and is subject to frequent interruptions.
Has a working presence in the community and is involved with participants, family members, personnel, visitors, government agencies/personnel, etc., under all conditions/circumstances.
Works beyond normal working hours and on weekends and holidays when necessary.
Participates in an on-call rotation
Subject to callback during emergency conditions (e.g., severe weather, evacuation, disaster, etc.).
Minimum Working Requirements
Must have reliable transportation, current valid Michigan driver’s license and car insurance.
Must be 18 years of age or older.
Must be fully vaccinated, or willing to be fully vaccinated against the following communicable diseases: Influenza, MMR, Varicella, Tdap/Td, Covid-19 and Hepatitis B.
