The Human Services and Public Health Department (HSPHD) is seeking a Social Worker who has an interest in providing outreach to individuals experiencing homelessness and working as part of Hennepin County’s Health Care for the Homeless multidisciplinary team. This position requires the ability to work independently as a Social Worker and as part of a larger team to provide wrap around services and case management to unsheltered clients living with HIV as part of Public Health’s Drug Related Infectious Disease (DRID) response. The position will provide outreach to individuals experiencing homelessness in Hennepin County, help support others on the team to offer access to programming, resources, and services as part of a person-centered approach.

Location and hours:  
This position is hybrid and will be performed primarily in the community, including shelters or drop-in centers, and remote as job duties require. Work hours will typically be Monday through Friday, with hours of 8 a.m. to 4:30 p.m., and occasional evenings as needed. No holidays or weekends required. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

In this position, you will:

Engage, establish trust and building rapport with the client. Assisting with transition planning when a client moves into permanent housing, ensuring connection to Ryan White Medical Case Management, and providing house stabilization services, mental health case management, and HIV care home services. Provide health education around relevant public health topics. Assist team and client with carrying out plan of care which can include activities such as: Establishing appropriate cares such as HIV specific clinic/treatment, primary care, mental health (MH), and substance use disorders (SUD). Enrolling and applying for appropriate benefits such as supplemental nutrition assistance program (SNAP), general assistance (GA), Minnesota supplemental aid (MSA), supplemental security income (SSI).Ensuring basic needs are met and assisting with resources and referrals.Completing the Coordinated Entry Assessment, referrals to other housing programs and assisting clients with reducing barriers to housing such as legal issues and obtaining vital documents.Assisting clients with transportation to appointments. Assisting clients with navigating complex social systems. Need to have: One of the following:Bachelor's degree or higher in social work, psychology, sociology, human services, or an equivalent field.Licensure as a social worker by the Minnesota Board of Social Work A valid driver's license and access to reliable transportation to travel throughout the community, including shelters or drop-in centers. Nice to have: Experience:Using EPIC and other data systems.Working with people living with HIV.Working with people who are unsheltered.Establishing appropriate cares such as HIV specific clinic/treatment, primary care, MH, and SUD. Providing health education around relevant public health topics.  Knowledge of:Unsheltered homelessness, SUD, mental illness, and harm reduction principles and practices. HIV, DRID, and HSPHD programs.SNAP, GA, MSA, and SSI programs. Ability to:Engage and establish trust and building rapport with clients.Assist the team and client in carrying out a person-centered goal plan.
