Job Responsibilities:

The following represents some major areas of concern in Social Worker.
  • Develop and implement individualized care plans in collaboration with clients their families and other members of the interdisciplinary team.
  • Provide counseling and support to individuals dealing with a range of issues such as mental health concerns substance abuse domestic violence and housing instability.
  • Maintain accurate and uptodate records of client interactions progress and outcomes in compliance with organizational policies and regulatory requirements.
  • Collaborate with other professionals such as medical professionals therapists and educators to ensure a holistic approach to client care.


Requirements
  • Master s Degree in Social Work (MSW) from an accredited program.
  • Current state licensure as a Licensed Social Worker (LSW) in Illinois.
  • 2 years of experience in providing social work services.
  • School clearly wants someone who has experience in School setting and is very well versed with IEPs 504 plans.
