The therapy staff and interdisciplinary team strive to create a comprehensive care plan that is unique to each short-term, outpatient, or long-term patient. We are committed to providing care so every resident can reach peak independence and functionality. Our dedicated therapy team has a proven track record of getting results

* *Responsibilities: * *

- Conduct comprehensive assessments to develop personalized treatment plans for patients.

- Administer therapeutic activities and exercises to enhance patients‘ functional abilities.

- Educate patients and their families on physical therapy‘s plan of care

- Collaborate with interdisciplinary teams

- Maintain accurate documentation of patient progress and treatment plans.

* *Qualifications: * *

- State licensure as a Physical Therapist.

- Excellent communication skills and ability to work effectively in a fast-paced environment.
