BASIC FUNCTION

The Supportive and Palliative Care (SPC) Service ARNP/PA Hospitalist will address the needs of patients with life limiting or serious illness when requested by referring providers. The Hospitalist will function as an integrated member of the SPC team; independently evaluate inpatients and make recommendations to improve care; exercise autonomy in medical decision-making; focus on the care of hospitalized patients to coordinate care; improve symptom management; address the needs of patients and families and optimize the utilization of resources across all health care settings.

KEY AREAS OF RESPONSIBILITY

Provision of Medical Care; Collaboration with Care Team:

Respond to Support and Palliative Care Service consultations.

Communicate with physicians who are caring for the patient, review of the medical record, assess patient and family needs for palliative care, including symptom management, psychosocial distress, spiritual suffering, and hospice.

Assist patients in identifying goals of care and provide support, counseling, and information and spiritual care as needed.

Collaborate with team members to ensure appropriate allocation of patient assignments to optimize effectiveness, maintain self-care and reduce compassion fatigue and burnout.

Assure completion of documentation and billing as per CCOM Internal Medicine department standards.

Effectively communicate with the primary medical service and inpatient nursing staff to coordinate care.

Serve as a consultative resource to staff; providing expert knowledge and behavioral skills to change clinical practice and patient outcomes.

Collaborate with internal and external agencies to assess financial and reimbursement issues.

Provide continuity by scheduling follow-up contact and assessment of patients in the SPC outpatient clinics.

Assist with the development and revision of standards of care for the Support and Palliative Care patient population.

Work collaboratively and professionally with other medical personnel and health professionals in the execution of patient care.

After communication with referring service and review of the medical record, assess patient and family need for palliative care, including symptom control, psychosocial needs, spiritual suffering and hospice or home care as appropriate

Facilitate communication between SPC Consultation Service and primary medical service and inpatient nursing staff to plan, deliver and evaluate care.

Maintain effective working relationships with faculty, staff, students and the public.

Intermittently function in the outpatient setting.

Perform other duties as assigned.

Education/ Health/ Promotion/ Research:

Assess, develop, and evaluate patient and family educational material related to palliative care and symptom management issues.

Serve in an educational, consultative and resource capacity to UIHC staff.

Participate in outreach activities to provide educational programs to external agencies or community groups upon request.

Foster professional development of medical, pharmacy, nursing, and advanced practice provider students, residents, and fellows.

Assist with planning of conferences related to Support and Palliative Care.

Administration/ Human Resources Management:

Provide leadership for and participate in appropriate University of Iowa Hospitals and Clinics (UIHC), College of Medicine, Department of Nursing meetings and projects and quality improvement projects.

Incorporate trends in palliative care into UIHC program development.

Participate in and assist with strategic planning.

Perform other special projects and tasks as assigned.

Interact with all staff, patients, and the public in a professional and cooperative manner.

Nurture and develop collaborative multidisciplinary relationships.

Assume responsibility for maintaining clinical expertise consistent with patient
population needs, and current nursing and medical practice.

Develop and maintain basic computer skills.

Maintain membership and participation in professional organizations.

Maintain contact with groups that influence the healthcare of this patient population.

Compliance with Internal Policies and External Regulations for Safe and Effective Care Delivery and Health Profession and Education:

Collaborate with the interdisciplinary team and various UIHC and COM departments and offices in the development of mechanisms used to monitor patient outcomes, and develop quality improvement plans.

Support the mission of UIHC by ensuring that the Support and Palliative Care Service is in compliance with rules and regulations set forth by regulatory agencies.

Education/License Requirements:

Physician Assistant (PA): Must comply with registration and licensure requirements as specified by the Iowa Board of Physician Assistant Examiners which include graduation from an accredited PA educational program and/or certification by the NCCPA.

Advanced Registered Nurse Practitioner (ARNP): A Master's degree is required. A Master's degree in Nursing is preferred. If a Master's degree is in a related field, a Baccalaureate degree in Nursing is required.

ARNP - Current certification in specialty area and licensure as ARNP in state of Iowa is required.

ARNP - Current license to practice nursing in the State of Iowa is required.

Required Qualifications:

Reasonable (1-3 years) experience in area of palliative care, oncology, chronic illness management, or geriatrics is required.

Excellent written and verbal communication skills are required.

Ability to work 8 weekends per year is required.

Ability to work one to two University holidays per year is required.

Demonstrated knowledge of effective strategies for working with faculty, staff, and students from a variety of backgrounds and perspectives

Desired Qualifications:

Experience providing overall medical care and provide medical management for patients in an inpatient setting is highly desirable.

Experience exercising autonomy in medical decision-making and services provided is highly desirable

Experience managing length of stay, improve resource utilization and decrease costs to patients and the institution while providing personalized care is highly desirable.

Experience in the provision of palliative care is desirable.
