*Now Staffing Physical Therapists for Rehab Department in state-of-the-art SNFs throughout NYC and Long Island.*

Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

*Hourly Pay Rate: $44-$46 Hourly (W-2), $57 Hourly (1099)*

*Physical Therapist Job Duties:*

· Provide efficient evaluations and create individualized treatment plans for PTAs to follow.

· Supervise PTAs daily.

· Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.

· Monitor and document patient progress with the use of innovative EMR software.

*Physical Therapists employed by Flagstar Rehab will receive:*

*Physical Therapist Requirements:*

· Current NYS PT License and Registration

· CPR Certification

· EMR Competence

Meets all State and Federal requirements for US employment in a Nursing Facility

· Malpractice/Liability Insurance (if applicable)

Job Types: Part-time, Per diem

Pay: $44.00 - $57.00 per hour
