We are hiring multiple Community Based Care Surveyors (Client Care Surveyor) to join our Community Based Care (CBC) team in Tigard, Oregon.

Employees who are not yet a PERS participating member, will receive the 6.95% pay increase and begin the 6% employee contribution upon participating in PERS.
Client Care Surveyor (AA): $5,232 – $8,024
Client Care Surveyor (AP): $5.598 – $8,581

What you need to qualify
Three years of experience in areas related to long-term care, nursing, or nursing home administration AND One of the following:
A bachelor's degree of Health Science, Gerontology, Pharmacy, Healthcare Administration, Long Term Care Administration
OR
A license as a Registered Nurse
OR
A license as a Long-Term Care Facility Administrator with a Bachelor’s in health service
Note: LPN certification and/or experience will not meet the minimum qualifications.
Essential attributes
We are looking for candidates with:
A minimum of three years of experience related to nursing, nursing home administration, long term care administration, or assisted living.
Rules/Regulations: Basic knowledge of Oregon State laws, rules, and regulations related to the provision of long-term direct-care programs and facilities.
Critical Thinking: Skill in exercising independent judgment and decision-making in the application of laws, rules, policies, or procedures.
Teamwork: Demonstrated ability to work successfully both independently and cohesively with a team.
Communication: Solid writing skills using the conventions of report writing and skill in communicating verbally with a variety of medical professionals, care provider management and staff, resident relatives, and personnel or other agencies, sometimes under tense or hostile conditions.

Your duties at a glance
As a Community Based Care Surveyor, you will:
Conduct state licensing surveys according to state protocols, guidelines, and time frames.
Review facility survey history, complaint investigation reports, and data reports.
Observe details of residents’ quality of care and life, including staff assistance with activities of daily living, staff interactions with residents, nutrition, activities provided, and environment.
Interview residents, families and facility staff.
Review resident medical records and facility policies and procedures.
Evaluate conditions, using OARs, state guidelines, and individual professional judgment to validate and determine findings of potential non-compliance.
Perform on-site inspection of the long-term care facility, including an inspection of the kitchen and other areas where food is prepared for residents.
Discuss findings with survey team in a consensus process to determine levels of compliance or non-compliance.
Write report of findings, using specified computer software program.
Review facilities plan of correction (POC) for deficiencies.
Conduct revisits as needed, to determine that facilities have corrected deficiencies cited at surveys.
Provide technical assistance to facility administration and/or staff for rule interpretation, or for raising issues of concern.
Prepare testimony, including affidavits for hearings and court proceedings.
Participate in unit or agency committees or projects for purposes of internal quality improvement (QI), or to assist with conducting relevant research to contribute toward enhancing residents’ quality of care and life in CBC facilities.
Assist with orientation, training, and mentoring of new surveyor staff.
Make presentations to civic organizations or other special interest groups.
Working conditions
The majority of your work will be conducted in the office and community.
Trail service and training will take place in Tigard, Oregon for 9 months.
Travel frequently, often overnight, to counties throughout Oregon up to two weeks out of the month.
Work daily within busy and varying long-term care healthcare facility settings.
Lift and carry up to 20 pounds throughout each day.
Irregular work schedules and frequent schedule changes.
You may interact with a wide range of people including some who have experienced trauma and exhibit challenges in controlling their emotions.
Walking into stressful situations that may require quick decision-making skills to ensure safety of those being serviced and own physical safety. This may also be trauma inducing.
Your office work location will be Tigard.
You will be a mandatory reporter of abuse and neglect of children, seniors, and other adults with disabilities.
