24 bed Skilled Nursing Facility

 

 

 

 

Under the direction of a registered nurse, the licensed practical nurse participates in the planning, implementation, and evaluation of nursing care, within the objectives, standards, and policies of the department and within the parameters of his/her preparation.  May also acted as the charge nurse.

 

EXAMPLES OF DUTIES:

 

The duties listed below are examples of work typically performed by an employee in this position.  An employee may not be assigned all duties listed and may be assigned duties which are not listed below.  Marginal duties (shown in italics) are those which are least likely to be essential functions of this position.

 

1.      Provide direct and individualized nursing care by planning, organizing, and directing the nursing functions of residents and maintain acceptable standards of resident care.

  1. Delegates to other personnel direct care duties consistent with their licensure, certification, and competency and as allowed in the practice setting.

3.      Complete admission evaluation and initial care plan within in 24 hours of new admission.

4.      Assist the RN with planning, implementation, and evaluation of nursing care plans and nursing care.

5.      Make changes to care plan as needed.

6.      Assist in preoperative, postoperative, and relevant health teaching for resident and family.

7.      Participate in staff meetings, sharing knowledge, information, and ideas among team members.

8.      Prepare, administer, and record medications in accordance with the policy regarding the administration of medications by a licensed practical nurse.

9.      Participate in emergency care.

10.  Communicate with the RN regarding resident’s physical and mental conditions.

11.  Documents information concisely, accurately, and completely.

12.  Identify and utilize appropriate channels of communication.

13.  Check residents into an exam room, obtain vital signs as needed and assist with exams per physician/PA discretion.

14.  Perform other related duties as assigned.

 

QUALIFICATIONS FOR EMPLOYMENT:

 

Knowledge and Ability:

 

Knowledge of

 

▪         guidelines of the Nurse Practice Act;

▪         resident rights and reporting requirements for abuse, suspected abuse, and unwitnessed injury;

▪         assessment and documentation procedures and responsibilities;

▪         procedures to take and record vital signs;

▪         reporting responsibilities;

▪         proper resident lifting and transfer techniques;

▪         colostomy and ileostomy care;

▪         oxygen administration;

▪         handwashing procedures;

▪         denture care;

▪         intake and outtake measurement procedures;

▪         communication and organizational skills;

▪         Hospital and department policies and procedures

 

Ability to

 

▪         organize workflow, respond to emergencies, set a professional example and note detail

▪         administer specified medication, orally or by subcutaneous or intermuscular injection, and note time and amount on residents' charts;

▪         observe residents and report adverse reactions to medication or treatment to medical personnel in charge;

▪         dress wounds, give enemas, douches, alcohol rubs and massages;

▪         apply compresses, ice bags, and hot water bottles;

▪         collect samples, such as urine, blood, and sputum, from residents for testing and perform routine laboratory tests on samples;

▪         assemble and use such equipment as catheters, tracheotomy tubes, and oxygen suppliers

▪         assist residents transfer and mobility;

▪         answer call lights;

▪         make beds;

▪         provide resident hygiene;

▪         prepare or examine food trays for prescribed diet and feed residents;

▪         monitor resident food input/output;

▪         record information;

▪         comply with hospital and safety policies and procedures;

▪         comply with Resident Bill of Rights and Employee responsibilities;

▪         work overtime, adjust to changes in days off and shifts on occasion if necessary;

▪         be flexible and able to work in an environment that has frequent and multiple interruptions, which is stressful at times;

▪         provide genuine caring for and interest in elderly and handicapped people

 

Experience and Training:

 

Any combination in training, education, and experience that would provide the knowledge and abilities.  A typical way to gain the required knowledge and ability is:

 

Possess and maintain a current LPN license in the State of Nevada, maintain current certification in CPR and participate in annual mandatory training. 

 

Current certification in IV therapy preferred.

 

PHYSICAL REQUIREMENTS:

The physical requirements described here are representative of those that must be met by an employee to successfully perform the essential functions of the job.

Dexterity, coordination, and vision to use a keyboard and video display terminal.  Strength and stamina to bend, stoop, sit, stand, and walk for extended periods of time.  Some bending, reaching, squatting, and stooping is necessary.  The manual dexterity and cognitive ability to procedures, care plans and tasks as assigned.  The ability to communicate via telephone and in person.  Mental stamina to deal with emotionally difficult situations.  Moderate lifting (up to 50 pounds) and use of mechanical lifts as required or needed.

WORKING CONDITIONS: 

Work is performed under the following conditions:

Work environment is generally clean and comfortable.  Lighting conditions are typically good.  Frequent interruptions to planned work activities occur.  Changes in days off and shifts are possible and incumbent will be required to be available on short notice.  Exposure to infectious diseases and bodily fluids is common.  It is common to engage with family members of residents during emotionally difficult periods of time.
