We have a variety of schedules to support work life balance

In this role you will:

  • Develop long term meaningful relationships with the patients and their families
  • Provide hightech nursing care to technologydependent patients in their homes
  • Collaborate with the healthcare team to ensure comprehensive patient care
  • Perform assessments administer medications and educate patients and families
  • Utilize innovative technology to monitor and manage complex medical conditions

We are looking for someone who:

  • Holds a current LPN license in Vermont
  • Two or more years of directly related home/public health hospice and/or maternal/child health nursing experience
  • Previous home care experience a plus
  • Possesses strong clinical skills and a commitment to patientcentered care
  • Demonstrates excellent communication collaboration and critical thinking skills
