Summary

This position is located in the Division of Clinical Services, Pharmacy Branch, Northern Navajo Medical Center, Shiprock, NM. The purpose of this position is to serve as a Antimicrobial Stewardship Ambulatory/Inpatient Pharmacist. The incumbent is primarily concerned with maintenance of the Antimicrobial Stewardship Program for the Service Unit, assisting with meeting Indian Health Service Chapter 39 and The Joint Commission(TJC)/Centers for Medicare & Medicaid Services (CMS) standards.

Duties

The incumbent is responsible for providing pharmaceutical care to patients seen in the hospital and in ambulatory areas. The incumbent strives to consistently provide pharmaceutical care to patients and optimizes patient care outcomes and functions at the highest level of clinical practice, works independently under their scope of practice as defined by the Northern Navajo Medical Center (NNMC), to directly care for patients. Utilizes prescriptive authority to dispense drugs prescribed by credentialed providers and advise them and patients on the selection, dosages, interactions, and side effects of medications. Monitor the health and progress of patients to ensure the safe and effective use of medication. Performs drug and component compounding. Serves as a liaison between Infection Control Service, Antimicrobial Stewardship Program, Pathology and Laboratory Service and Pharmacy Service Serves as consultant in planning, monitoring and evaluating drug programs or regimens by evaluating medication use patterns and outcomes for patients in hospitals or managed care organizations. Leads the Antimicrobial Stewardship Program and is responsible for assisting in developing, implementing, maintaining the Antimicrobial Stewardship Program in compliance with TJO and the CMS Condition of Participation and improving overall antibiotic use at the facility. Works with Clinical Application Coordinators (CAC's) and/or Pharmacy Informaticist to update antimicrobial order sets and algorithms for the Electronic Health Record (EHR) as well as retrieve data from (EHR). Employee education/training of new hires, is ongoing. Develops staff skills and training plans. Assists in the development, enhancement, and operation of resident and student training programs. Assists in designing and completing research studies involving clinical pharmacist functions, quality assurance, drug use evaluations, or pharmacotherapy. The Antimicrobial Stewardship Ambulatory/Inpatient Pharmacist is a licensed pharmacist, preferably with an American Society of Health-Systems Pharmacists accredited residency, fellowship, and/or board certification or equivalent experience demonstrating competency in Infectious Disease. Collaborates with the infection Preventionist and the infectious Disease consultant on the implementation of the Antibiotic Stewardship Program. The incumbent has Advanced Cardiovascular Life Support certification. **Note: This is an Open Continuous Announcement; applicants will be referred when vacancies occur.
