Join Our Team as an Admissions Registered Nurse at VITAS Healthcare
The Admissions RN is a crucial member of the Interdisciplinary Team at VITAS Healthcare. In this role, you will be responsible for introducing VITAS services and philosophy to patients and families, ultimately facilitating the admission of suitable patients.
- Participate in information gathering, analysis, education, and form completion during the admissions process
- Evaluate patients for hospice suitability by reviewing medical records
- Obtain agreement for the admission of eligible patients and families
- Analyze data collected by the LPN to assist in obtaining admission orders
- Develop strong relationships with referral sources to uphold VITAS' quality standards
- Maintain clinical and professional competence
- Utilize interpersonal skills to educate and address queries regarding hospice and palliative care
Key Responsibilities Include:
- Knowledge of intake, admission, and transition to care processes
- Effective communication with patients, families, physicians, and colleagues
- Understanding of clinical disease manifestations and VITAS-specific clinical knowledge
- Proficiency in VITAS information system and mobile technology platforms
- Reliable transportation
Qualifications
- Current R.N. license in the state of practice
- Prior experience in hospital, physician practice, LTC, or ALF settings
- Demonstrated Presentation Skills
- Previous sales or service experience is advantageous
Education:
