This position directs the overall administrative and clinical operations of SWCMHS Community Resources and Special Projects unit that serves over 500 clients annually. The position serves as the liaison between multiple systems of care in DMHAS Region 1 including the affiliate network of programs, SWCMHS inpatient and outpatient services, statewide utilization management, DMHAS Community Services Division, CVH, WFH, DOC, and the judicial system. Additionally, this position ensures the quality of services in the contractual network of agencies through the development of collaborative partnerships with affiliate providers that is based upon a shared commitment to working together with the overarching goals of enhancing program services for the individuals being served and that services in the SWCMHS managed Service System are aligned with DMHAS Departmental priorities. It also participates in and facilitates the utilization management of the current SWCMHS MSS service system including the development of additional residential and service options for individuals transitioning from an inpatient to and outpatient level of care. PURPOSE OF JOB CLASS (NATURE OF WORK)

In the Department of Mental Health and Addiction Services this class is accountable for directing a comprehensive behavioral health outpatient clinic and community service program.

EXAMPLES OF DUTIES

The overarching goal of the multiplicity of job duties listed below is to facilitate the linking, increase the accessibility, enhance the quality, and expand the services for individuals transitioning between systems of care to resources and programs in region’s the affiliate network. The duties include, but are not limited to: Attends and participates in local and state-wide UM meetings to facilitate the transition of individuals served by the long-term state inpatient facilities to community-based residential and service affiliate programs; Understands the statewide needs and direction of individuals transitioning from inpatient to community care as a basis for regional service development and expansion; Partners with affiliate providers to develop proposals to expand affiliate services aligned with departmental priorities; Manages and oversees referrals to the residential programs in the MSS at the MRO GH, Supervised, and Scattered site level of care; Manages and oversees the placement of individuals in the 8 Transitional Care beds at PVS; Serves as the LMHA point of contact for all inquiries concerning the Mental Health Bed registry for region 1 affiliate agencies; Participates in local and OOC case conferences to coordinate and consult regarding individuals with significant clinical needs to access services and residential supports in the affiliate network of care; Provides consultation to SWCMHS clinical teams regarding available resources in the Managed Service System (MSS) to supplement team-based outpatient clinical and case management services; Functions as the LMHA liaison for individuals hospitalized at CVH transitioning back to community-based services in region 1 involving participation in the individual’s TPRs and coordinating care with the CVH inpatient team, affiliate providers, and referral to SWCMHS outpatient services; Participates in the quarterly UM meetings for the 2 Intensive Mental Health programs (Madison and Walton) in region 1; Attends and facilitates the monthly collaborative meetings between SWCMHS outpatient teams and the 2 largest affiliate providers (MHC and RNP) to facilitate collaborative planning and problem solving for individuals served in both systems of care; Provides administrative and clinical supervision to the SWCMHS Managed Service System (MSS) and Special Projects Behavioral Health Program Manager; Supervises SWCMHS forensic services and provides coordination of care for individuals transitioning between the judicial system, SWCMHS outpatient, and WFH civil, PSRB, and 5456D services; Provides clinical consultation and group supervision to the regional Jail Diversion clinicians who serve as TL supervisors for all the PSRBs served in DMHAS Region 1 and are responsible for coordinating housing and outpatient services for end of sentence individuals leaving DOC, transitioning 5456D individuals from WHF, and managing placement and referral to the JD beds at designated affiliate Residential Programs; Provides consultation regarding MSS resources and programs that match needs of individuals transitioning from forensic systems of care (DOC, PSRBs); Provides respite back-up at PVS for forensic individuals transitioning from the WFH restoration units when the local JD beds are full; Participates in the monthly collaborative meeting between inpatient PSRB staff and SWCMHS TL supervisors to coordinate care; Participates in Critical Incident Debriefings as necessary; Complies with ethical standards and DMHAS work rules; Maintains client and program safety; Utilizes an electronic health record, computer technology and data to perform job functions and inform management and supervisory practices; Performs safe work practices; Observes all confidentiality and client’s rights regulations; Provides CPR and first aid as necessary; Performs other duties as required. KNOWLEDGE, SKILL AND ABILITY

Considerable knowledge of program principles and practices;  causes, treatment, prevention, recovery and management of crisis in adult psychiatric patient population;  community resources; Knowledge of and ability to apply management principles and practices; relevant state and federal laws, statutes and regulations; resources available for client care;  principles and practices of patient advocacy; causes, treatment and preventions of drug and alcohol addiction and other psychiatric illness;  vocational, educational and rehabilitative services; budgeting and financial reporting principles and techniques related to outpatient and community based programs;  Considerable interpersonal skills;  oral and written communication skills.

MINIMUM QUALIFICATIONS - GENERAL EXPERIENCE

Nine (9) years of professional experience in behavioral health treatment programs.

MINIMUM QUALIFICATIONS - SPECIAL EXPERIENCE

Three (3) years of the General Experience must have been at a managerial level with responsibility for fiscal and administrative management of behavioral health programs.

NOTE:

Managerial experience is defined as formulating program goals and objectives, developing and implementing program procedures, initiating program policies and developing and/or monitoring a budget. Behavioral Health is defined as the integration of mental health and substance abuse programs.

PREFERRED QUALIFICATIONS

Possession of a clinical license in social work (LCSW), psychology, or professional counseling (LPC) Experience with the clinical operations of a Community Behavioral Health Service System Experience overseeing Private Not for Profit service contracts, developing programs, writing proposals, and overseeing and developing program budgets Experience developing and providing high quality services for individuals with serve and persistent behavioral health disorders and chronic medical diagnosis Experience acting as a liaison between multiple systems of care including: Private Not for Profit programs, DMHAS outpatient services, statewide services/utilization management, Community Services, State Operated DMHAS Psychiatric Hospitals, Department of Corrections, and/or the Judicial System Experience participating in and facilitating utilization management meetings Experience conducting program audits and fidelity reviews Experience providing consultation to clinical teams regarding available community resources Experience providing administrative and clinical individual and group supervision to forensic clinicians Experience with the requirements of the joint Commission and other regulatory requirements Experience working in a union environment Experience using an Electronic Medical Record
