The QIDP Coordinator works under the direct supervision of the Assistant Director of Programs (ADOP) and has the primary responsibility for providing leadership and directions to QIDPs to assure the delivery of quality program services that promotes dignity and respect for the individuals served. The QIDP Coordinator is responsible for working cooperatively with other divisions and departments and for ensuring harmonious working relationships within his/her unit among staff, family members and other facility employees. The QIDP Coordinator is responsible for planning and coordinating the use of all departmental resources. The QIDP Coordinator is responsible for training and coordination of QIDP services and data processing services. This position at times may be required to manage a partial QIDP caseload. The QIDP Coordinator performs complex consultative serves and provides technical assistance to Interdisciplinary Team (IDT) members. Some flex time, overtime, evening and weekend work will be required.
Essential Job Functions:
Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned. May serve in an on-call status on a rotating schedule as required to meet operational needs of the facility.
Professional Oversight/Monitoring – The QIDP Coordinator is responsible for providing professional oversight and monitoring of all QIDP related duties ensuring that professional practices are in compliance ICF/IID Regulations and the conditions of the DOJ Settlement Agreement. The QIDP routinely monitors the conduct of Individual Support Plan (ISP) meetings and the resulting written product and provides feedback to the QIDPs to improve their performance. Ensures that QIDP services are delivered in a consistent, standardized manner across the facility.

Personnel Activities – The QIDP Coordinator will oversee and participate in the screening, interviewing and selection of all QIDP staff. This position will monitor the work performance of the QIDPs and coordinate the performance evaluation of all QIDPs, including disciplinary action when appropriate. The position will provide supervision, leadership and direction to ensure a well-trained, knowledgeable and competent QIDP Workforce.

Policy/Procedure Maintenance/Development - The QIDP Coordinator is responsible for developing, revising and maintaining policies/procedures relating to QIDP duties and the Individual Support Plan development, implementation and monitoring process. The QIDP Coordinator ensures QIDPs are knowledgeable of and effectively implementing these practices.

Training – The QIDP Coordinator coordinates and provides oversight to ensure orientation and training to newly hired QIDPs and ongoing training on an as needed basis for all Center QIDPs. The QIDP Coordinator serves as the Center’s ISP discipline coordinator and provides training, as appropriate and when requested, to all staff regarding the ISP process and any revisions or process changes.

Quality Assurance - The QIDP coordinator performs complex training, monitoring, tracking and trending of data regarding the outcomes for individuals. This includes working in coordination with other discipline heads to develop processes and practices to meet the designated services outcomes. The QIDP Coordinator is designated areas of responsibility and will implement processes and practices to successfully meet the outcomes of those sections. Additional responsibilities include the collection and analysis of data related to QIDP activities and the development of plans of correction, submission of evidence in a timely manner, and participation in the Quality Assurance/Quality Improvement Council.

Service Improvement/Coordination - The QIDP Coordinator works with other departments to ensure service coordination. The QIDP Coordinator identifies problems and issues that affect the provision of services, coordinates efforts to develop solutions and follows-up until problems are resolved. Assists with other assignments as necessary including serving on multiple Center committees and work groups as assigned. The QIDP Coordinator performs after-hours monitoring, and other monitoring duties as assigned. The QIDP Coordinator performs special projects and other duties as assigned.

Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.
