Summary
This position provides comprehensive nursing services to individuals, families, and communities within a designated geographical area. Servicesare provided to all age groups with focus on health promotion and disease prevention The health program is carried out in homes, offices,
clinics, schools and the community.
Duties
Conducts community assessments to identify physical, mental, social and/or environmental deviations from the normal, responses and attainments of outcomes, using nursing skills of assessment, nursing diagnosis, planning intervention and evaluation. Assesses patient needs and then develops plans of care. Consults with health care team members, including participating in case management with health care providers. Counsels patients and/or families in the home, community, school and clinic regarding acute and chronic conditions and the application of health promotion and disease prevention practices. Participates in Performance Improvement activities, facilitates and promotes Government Performance Results Act activities, and is involved in facility accreditation activities.-
