Summary

The Facility Telehealth Coordinator/Nurse Manager manages care across the health care continuum. He/she
works closely with primary, specialty, geriatric and behavioral health care providers and other health care
professionals and team members, other clinics, internal services, and community agencies.

Duties

The Nurse Manager Registered Nurse demonstrates: Executes position responsibilities that demonstrates leadership, experience, and creative. approaches to management of complex patient care. Nurse Manager is an EXPERT nurse, "with an extensive background of experience. The Nurse Manager registered nurse demonstrates performance and leadership that is broad enough to improve the care for a group of patients. The Nurse Manager is responsible for the documented outcomes at the program or service level. Program or service level outcomes must be broad and complex and can be
demonstrated at any organizational level within a facility, VISN, or VACO. Program is defined as a substantial coordinated group of activities impacting patients and/or employees. Career path roles may include, but are not limited to clinical nurse leader, nurse coordinator, nurse educator, expert clinician, nurse manager, nurse practitioner, or researcher. Leading and organizing delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning Using advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating outcomes Functioning as an expert in clinical practice and/or areas related to the assigned roles and responsibilities Systematically evaluating current practice, and formulating outcomes for groups of patients and/or organizational processes within an area of expertise Guiding, developing, and supporting staff from a leadership perspective Using professional standards of care, scientific evidence, and practice to evaluate programs and/or service activities. The Facility Telehealth Coordinator/Nurse Manager has programmatic responsibility for the VA staff, services, and/or processes within specified area(s) of assignment. He/she works collaboratively other Program/Service Leaders to ensure adequate staffing, competency of staff, resolution of personnel issues, and efficient operation of the integrated units. The FTC/Nurse Manager executes leadership that is characterized by substantial and continuous responsibility and accountability for population groups or integrated programs that cross service and/or discipline lines and influence organizational mission and health care. Functions as an expert in clinical practice and/or areas related to the assigned roles and responsibilities. Systematically evaluates current practice and formulates outcomes for groups of patients and/or organizational processes within area of expertise. Guides, develops, and supports staff from a leadership perspective. Uses professional standards of care, scientific evidence, and practice to evaluate programs/service activities. Demonstrates grade-appropriate and consistent critical thinking skills. VA offers a comprehensive total rewards package:
