Summary
The primary responsibility of the Cancer Care Nurse Navigator is to coordinate the cancer patient's journey through the health care system, across disciplines and sites. This position is essential to the Cancer Program at the Ralph H. Johnson VA Medical Center, and all activities of the Nurse Navigator are dedicated to the patients with cancer and the cancer care team.
Duties
The primary responsibility of the Cancer Care Nurse Navigator is to coordinate the cancer patient's journey through the health caresystem, across disciplines and sites. This position is essential to the Cancer Program at the Ralph H. Johnson VA Medical Center, and all activities of the Nurse Navigator are dedicated to the patients with cancer and the cancer care team. He/she is a stable and reliable point of contact for both Veteran patients and providers and provides oversight of the patient's journey through the diagnostic and treatment continuum, ensuring that all steps in the process are scheduled, timely and completed, while tracking the overall treatment plan. He/she will follow select patients from the earliest suspicion of cancer through the continuum of cancer care including diagnosis, treatment, survivorship/surveillance, or palliation/end of life.The Nurse Navigator will interface with outside referral agencies, and ensure that their business and clinical practices are in compliance with regulatory standards, including but not limited to, the professional association for oncology nurses, (Oncology Nursing Society), The Joint Commission on the Accreditation for Healthcare Organizations, (JCAHO), the Commission on Cancer "Cancer Program Standards: "Ensuring Patient-Centered Care" (2020 edition), National Cancer Institute (NCI), AJCC (American Joint Committee on Cancer (AJCC) standards and staging guidelines, National Comprehensive Cancer Network (NCCN) Clinical Practice Guidelines and OSHA. Through his/her well-developed clinical experience and navigation expertise, the Nurse Navigator will anticipate and implement preventive patient care measures. He/she will ensure that the appropriate plan of care is implemented according to the standards of regulatory agencies, such as JCAHO, OSHA and the appropriate professional nursing organizations. The Nurse Navigator possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs, providing care as considered necessary. He/she provides guidance and education to patients and caregivers and collaborates with other professionals at all levels, including but not limited to: social workers, pharmacists, radiology staff, laboratory staff and the other nurses and physicians on the various clinical treatment teams (Medicine, Hematology/Oncology, Surgery, Primary Care, Palliative Care and/or Hospice teams). The plan of care and educational requirements are individualized and tailored specifically to support each individual Veteran patient's needs. He/she reports status of service needs and contributes to problem resolution and modification of processes as necessary. He/she may be required to participate in formal Process Improvement Teams using the Lean 6 Sigma process.
