Requirements:
Active Virginia license
Doctor of Physical Therapy (DPT) degree
CPR Certification
Responsibilities:
Responsible for the provision of therapy consultation evaluation and services to patients.
Provide comprehensive physical therapy services to patients ensuring their rehabilitation and wellbeing.
Maintain accurate detailed reports and records.
Complete reports or assessments/evaluations and progress notes utilizing facilitys approved documentation forms.
Perform care consistent with the scope of licensure and in accordance with the facilitys policies and procedures.
Work effectively with fellow medical staff and treatment team members.
Comply with all state requirements and facility policies and procedures.
Additional duties as assigned by facility.
