- Are you a registered nurse with a passion for providing high-quality services to patients and clinical teams? As a Care Manager RN, you will play a crucial role in ensuring a safe discharge for patients at Hoag Memorial Hospital in Newport Beach.
Working collaboratively with physicians, nurses, and community agencies, you will assess patients' needs and coordinate post-acute care to deliver exceptional patient care.
Join our Care Management team at Hoag and be part of a streamlined process that ensures timely, cost-effective, and high-quality care for patients from admission to post-discharge.
Key responsibilities include:
- Developing and implementing discharge plans by assessing clinical, psychosocial, and financial needs.
- Coordinating with the interdisciplinary team to gather clinical information for comprehensive discharge plans.
- Overseeing post-acute care arrangements for patients on ECMO or home ventilators.
- Collaborating with community resources to meet patients' clinical and psychosocial needs.
- Educating patients on available resources for short and long-term care plans.
- Documenting patient issues, contacts, and discharge plans in medical records.
- Participating in quality improvement processes and maintaining hospital standards.

Requirements:
- Current RN License in California.
- Bachelor's degree in nursing (BSN).
