Due to our continued growth , Outreach is looking to bring on additional motivated and passionate Therapists to work with the adult and geriatric populations.
Apply to join the collaborative and supportive Outreach team Qualifications of Physical Therapist:In Home - Masters degree a minimum requirement 1 year of experience working with older adults preferred.
Home care experience a plus.

Patients may or may not be home-bound, low or high functioning.We aim to bridge the gap between a home health agency and an out patient clinic.
We provide all treatment in patients' homes, Senior Residences Assisted Living Facilities, or office setting Responsibilities of Physical Therapist:
