Duties are but not limited to:
Delivers fundamental knowledge-based care to assigned clients while developing technical skills.
Assesses, plans, implements, and evaluates care based on age-specific components.

Assumes responsibility for the coordination of care focused on patient education, self management, and customer satisfaction throughout the continuum of care.

Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team.

Performs the appropriate documentation that reflects assessment of patient needs, interdisciplinary treatment plans, provides education and support to the patient and their families, plans clinical interventions, and evaluates patients' responses to treatments.

Must have the ability to effectively interact with Veterans and their families as well as other support systems with the hospital.

Responsible for the coordination of continued professional nursing competence through education, orientation and periodic competence assessments that shapes and guides the nursing profession.

Educates staff on current nursing practices and treatment modalities of patients within Nephrology services.
Coordinates training activities and in-services for all nursing staff and affiliate students at the unit.
The cultural, racial and ethnic diversity of all staff will be considered when dealing with staff and patients.

Provision of these educational offerings will provide a positive impact on direct patient care and for the delivery of quality nursing care to a culturally diverse veteran patient population.

The standards of practice for Nurse Clinicians include assessment, diagnosis, and analyses to determine target audience and learner needs, identification of educational outcomes, planning, implementation and evaluation.

Ongoing evaluation of educational activities is essential to maintain and enhance professional staff development.

The Clinicians use data, information technology and wisdom to support all areas of nursing and healthcare, including, but not limited to, the direct provision of care, establishing effective administrative systems, managing and delivering educational experiences and enhancing participation in the VHA National Kidney Program for monitoring and monthly reporting of outcome data and quality indicators via the Dialysis Data Module.

Monitors and reports to IPEC, outpatient dialysis access related infections and report findings to Infection Control Department.
Assists with the QAPI meeting preparing reports and coordination of outcomes presenting PI findings as requested.

Performs all required elements of a nursing assessment including but not limited to physical examination, assessment of readiness to learn, psychosocial assessment, functional assessment and utilization of patient/family specific assessment scales as required.

Responsible for direct patient care to peritoneal and hemodialysis patients in the dialysis unit or ICU, ensuring that the appropriate plan of care is implemented according to the standards of regulatory agencies for safe patient staffing, such as JC, OSHA and VHA Directives.

Manages patient care in an astute manner, which protects patient's safety and promotes positive patient outcomes.

Possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to their age specific needs, providing care as considered necessary.

Provides guidance and education to patients and caregivers and collaborates with other professionals at all levels including but not limited to the other nurses and physicians in the Dialysis Unit, as necessary.

Reports status of service needs and contributes to problem resolution and modification of processes, as necessary.
Provides on-going patient support, oversight and education related to vascular access coordination.
