Facilitate collaborative clinical cases to meet complex health needs across the care continuum with the goals of decreasing fragmentation of care and enhancing the patient's quality of life while containing costs.
Coordinate with Managed Care Contractor for clinical case management issues when beneficiaries require the majority of their care outside the Medical Treatment Facility (MTF).
Provide robust and comprehensive clinical case management, care coordination, and discharge/disposition services.Coordinate care and collaboration between multiple militaries and/or civilian (purchased care) healthcare agencies.
Educate the patient and family on the clinical case management process, healthcare and treatment options, community resources, insurance benefits, and psychosocial concerns so informed decisions can be made.
