Nurse (Case Manager)

Incumbent ensures patients at the Albuquerque Service Unit receive quality, patient-centered care that is timely, appropriate, cost-effective, sensitive and coordinated to affect desired clinical outcomes.
Provide holistic and comprehensive case management services to all patients that include: intake assessment, benefit assessment, goal setting, long-term care plan development, progress monitoring, advocacy and referrals.
Serve as the primary contact between referral health care agencies, treatment facilities, patients, patient's families to ensure delivery of effective and appropriate treatment according to established plan of care.
Develop patient care plans with the care team according to clinical practice guidelines with the patient, family, caregiver(s) and significant other(s).
Provide formal and informal health education to patients and family on a one-to-one basis and/or to groups.Collect, trend and analyze outcome data to be used in identifying best practices and targeting performance improvement solutions.
Initiate therapeutic interventions and administers prescribed medications.
