Included but are not limited to:


Responsible for providing competent, evidence-based specialty care to patients with oversight of licensed vocational nurses/nursing assistants as appropriate to the clinic setting.

Provides patient care, care coordination, health promotion, health education and coaching on wellness, disease prevention, and chronic care management.

Manages secure messaging to facilitate care coordination.
Assists in directing the provision of nursing education, orientation, competencies and providing quality improvement and enhancing outcomes utilization.

Integrates specialty care knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex outpatient situations.

Validates understanding of the rationale for specialty care processes and decisions.
Questions clinical practices for the purpose of providing evidence-based care.
Demonstrates the ability to cope with and manage competing priorities.
Demonstrates competence in specialty care practices and decision-making, deliberate planning, and critical thinking skills.

Work Schedule:
Full Time, Monday - Friday, 7:30 am - 4:00pm

This position is covered by a special rate.
Current and former Federal employees must submit copies of their most recent SF-50, (Notice of Personnel Action). Pay:
Competitive salary, regular salary increases, potential for performance awards
