Summary

Full Position Title: Geriatrics Case Manager, RN



Duties

Brief summary of the position

Consultation and management of Veterans with multiple comorbidities, dementia, frailty, and its associated complications who could benefit from an interdisciplinary team approach. Evaluate patients through traditional face-to-face appointments, telephone visits, and virtual modalities approved by VHA. The Geriatric Case Manager will support the Ambulatory Geriatric Evaluation Clinic through patient care and process management.

  • Act as the nurse navigator for the Geriatric Evaluation Program.
  • Independently complete pre-visit, initial, and follow-up patient assessments.
  • Provide patient, family/caregiver education and ongoing case management as part of the interdisciplinary Geriatrics Evaluation Team.
  • Collaborate with Geriatrics physicians on consult triage and ensure efficiency and accuracy in scheduling patient appointments.
  • Serve as the Geriatrics Service liaison with Patient Aligned Care Team (PACT) staff and be GE representative at staff meetings and committees.
  • Collaborate, lead, and collect data for quality improvement projects.
  • Proficiency with Microsoft Office 365 products required.
  • Ensure timely and accurate documentation and encounter completion.
  • Provide information about and referral to VHA, state, and community-based resources.
  • Ensure adherence to local and national consultation guidelines and directives.
  • Participate in team huddles.
  • Attend required monthly/quarterly program meetings.
  • Utilize appropriate telehealth and virtual visit modalities.

Work Schedule: Monday - Friday: 8:00 am-4:30 pm.

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
