General Description:
Seeking a diligent and compassionate Substance Use Care Coordinator to serve a crucial role in the ER Diversion program. This specialist will focus on individuals with high emergency room utilization rates, ensuring they receive comprehensive care coordination that extends beyond emergent care into less restrictive settings. The Care Coordinator will be pivotal in establishing connections to substance use, mental health, and primary care services. They will provide support for overcoming psychosocial barriers and facilitate seamless care transitions, including "warm handoff" services.

Key Responsibilities:
  • Develop tailored care plans within the first 30 days, focusing on individual needs and preferences.
  • Facilitate linkages to local substance use and mental health service providers, monitoring engagement and follow-up care.
  • Work directly with medical providers who manage medications for opiate and alcohol use disorders.
  • Serve as a liaison between inpatient and outpatient substance use provider teams.
  • Coordinate services, ensuring all assessments and interventions are well-documented in clinical charts.
  • Offer ongoing support to individuals and their support network members, empowering them to navigate through social determinants of health challenges.
  • Provide care coordination assistance aiming to remove barriers to recovery such as housing, employment, and transportation needs.
  • Engage with participants during post-emergent care to promote sustained recovery in community-based settings.
  • Provide and document assessments required by state authorities and healthcare agencies
  • Development and maintenance of care plans
  • Provide support for other staff in the ER Diversion Program Pilot and related programs.
  • Provide services under the aspirational ideal of "Unconditional Care for Everyone"

Requirements:

Formal Education:
  • Bachelor's Degree in Psychology, Social Work, Counseling, or related human services field

Work Experience:
  • Minimum of two years of experience in health care settings.

Required Licenses, Certifications, Registrations:
  • Valid driver's license required.
