Provides supportive services and case management to Veterans residing in Contract Emergency Residential Housing facilities.

Utilizes various treatment modalities of individual and group work, casework, community organization and community outreach activities to alleviate or remove problems that interfere with the Veteran's ability to achieve independence.

Monitors the clinical care and asses the compliance of the programs receiving payments, as described in the recipient's contract.

When necessary, the incumbent intervenes to facilitate compliance or corrective actions and, when appropriate, develops, facilitates and monitors plans of correction.

Provide oversight of the Contract Residential Services as defined by VHA policy.

This includes, but is not limited to, verification of Veteran's status and eligibility for participation in programming, maintaining admission and discharge dates of participants for billing purposes, reviewing and approving/disapproving readmissions to and extensions of care.

Serves as the Contracting Officer Representative (COR) for the Contract Emergency Residential Housing Program, upon completion of required training and certification.

The incumbent coordinates care and acts as an advocate on behalf of Veterans to ensure comprehensive service delivery and provides linkages for Veterans to VHA health care services, Veterans Benefits Administration (VBA) and community agencies.

Assists the supervisor with ensuring the Contract Emergency Residential Shelter program meets Commission on Accreditation of Rehabilitation Facilities (CARF), SSI/SSDI Outreach, Access, and Recovery (SOAR) and Joint Commission requirements.

Is a member of the CARF preparation committee and assists with associated administrative and quality management tasks.
Provide a wide range of services, including assessments, referrals, short-term case management, advocacy, and service coordination.

Provide a full range of social work services within commonly accepted standards of social work practice which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate.

Evaluate and assess Veterans at risk of danger to self and/or others and utilize the appropriate emergency channels within the VA system or local community dispatch agents to address the crisis.

Provide consultation services to other VA staff regarding the psychosocial needs of homeless Veterans and the impact of psychosocial problems on healthcare, adherence to treatment, and housing plans.

Maintain a level of productivity and quality consistent with the complexity of the assignment and consistent with Social Work standards, Joint Commission standards, and VA professional standards.


Perform timely accountability functions:
1) document all contacts with Veterans in the electronic health record charts; 2) complete statistical and other reports in accordance with VA CERS and respective professional discipline service policies.
Assist with, develop, and/or facilitate development and implementation of site-specific social work programming.
Participate in conducting team conferences to promote professional development, training, and coordination of service delivery.
Complete mandatory and service line related trainings as assigned or offered.

Support the mission, policies, and procedures of the Department of Veterans Affairs, the Veterans Health Administration (VHA), the appropriate Veterans Integrated Service Network (VISN), and the facility.

Serves as a mentor to other staff, particularly those at the entry level.

Provides clinical supervision for unlicensed staff and is viewed by colleagues as a senior clinician with advanced practice skills and expertise in the treatment of patients with homeless issues, substance abuse and mental health disorders.

Facilitate and collaborate in outreach endeavors as required by facility, program, and supervisor's direction.
