As a Social Worker MSW, you will work with a care team to provide supportive services to patients and their families. Assists Director to ensure all staff is providing quality care to patients and families from admission through discharge and aftercare. Primary Duties:

  • Assesses the patient's and family's psychosocial needs as related to the patient's terminal illness. Consults and collaborates with the interdisciplinary team in developing and implementing the comprehensive care plan.
  • Documents problems, psychosocial assessment, appropriate goals, care provided, interventions and patient, family, and caregiver response to each intervention. Assesses and refers for ongoing bereavement services.
  • Supports the patient unique cultural and spiritual beliefs with referrals to clergy or chaplain as requested. Enhances the responsiveness of the environment by connecting the patient, family and caregivers with community resources as needed.
  • Provides interventions for specific symptom relief (fear, grief, depression, anger, etc.). Screens for psychopathology and educates/intervenes accordingly.

Qualifications:

  • Master's degree in social work.
  • LMSW (Licensed Master Social Worker).
  • Healthcare experience preferred.
  • Experience in home health care preferred.
  • Completed hospice training program preferred.
