General Practice Physician Job with in Wilmington, DE

TeamHealth is hiring for a full-time 3.5 days per week physician to do primary care rounding in two skilled nursing facilities (SNF) and one assisted living facility (ALF) with facilities located in the Wilmington, Delaware, area. Great work/life balance with the ability to set up your rounding schedule as it fits into your life.
Job Duties:Primary care rounding in SNF and ALFManage chronic disease, diagnose illnesses and medical conditions, and implement plan of careCollaborate with clinicians, facility leaders and others regarding care and servicesBuild relationships with your patients, patients' family members, and facility staff to facilitate effective treatment deliveryProvide oversight, guidance, and mentorship to our nurse practitionersConduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of careRepresent TeamHealth on appropriate external levels engaging and establishing/maintaining relationships within the post-acute care communityCollaborate with leadership to review care coordination initiatives, develop collaborative intervention plans, and share ideas about network management issuesSkills:Excellent clinical skillsCritical thinkingExcellent communication skillsTeam playerStrong work ethicEmpathy, compassion and ability to connect and engage with your senior patients and familyHuman touchQualifications:1+ years experience in primary care requiredPost acute care experience a plusActive and unrestricted Delaware licenseActive and unrestricted Delaware DEA/prescriptive authorityBenefits:
