The Complex Care Case Manager (RN) - Addiction will work collaboratively with primary care providers and other members of the clinical care team, the Complex Care Case Manager (RN) - Addiction brings expertise in addiction and recovery works as a member of an expert team of professionals to provide patient-centered, outcome- driven care that promotes independence and dignity.


Responsibilities:
Provide intensive case management supports and related care coordination for a panel of patients with addictive disorders.
Work closely with MAT Clinic Team, behavioral health, and medical providers on a regular basis.

For MAT clinic:
Request Prior Authorizations and TARS for medication(s), etc., for patients to receive medications, etc., in a timely manner.
Conduct assessments and create tailored care plans for individual patients.
Ensure that all program reporting requirements are met.
Triage and respond to tasks/care needs.
Internal and external resource connection/referrals.
Track referrals and provide follow-up to ensure service delivery.
Assist identified patients to coordinate all necessary care.
Conduct home/community visits as needed.
Consult/coordinate with interdisciplinary care team, including behavioral health, care navigation, MAs, nursing, medical providers, and external partners.
Attend internal and external meetings (ie case conferencing, administrative).
Attending trainings such as health coaching, engagement techniques, etc.
Timely and accurate reporting of data as requested to MCC CM programs.
May travel between clinics or meet Patients in the community as needed.
Provide frank and considered opinions regarding case management services and quality improvement measures.
Provide services to satellite clinics as needed.
Other duties may be assigned.

Qualifications:

Education and Experience:
RN, CAC, or equivalent experience.
1-2 years' experience (and passion for) providing support to individuals with SUD highly preferable.
Knowledge of state regulations and legal/ethical standards related to patient rights, and client/patient confidentiality required.
Electronic Medical Records, EPIC experience desired but not required.

Required Skills and Abilities:


Must be fluent in English with the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.

Ability to write reports and business correspondence in English.
Bilingual skills - Must be able to communicate with patients and partners in Spanish.
Effectively present information and respond to questions from clients, customers, and the public in Spanish.
Strong preference for a candidate than can conduct clinical assessment interviews and case management interventions with patients in Spanish.
Position will require travel to multiple MCC sites in Marin County.

Computer Skills:


To perform this job successfully, an individual must be computer literate and knowledgeable in Microsoft Office software programs, and how to conduct searches on the Internet.

CPR Training Current
Valid CA Driver's License

Physical Requirements:
Prolonged standing, walking, and prolonged periods sitting at desk and working on a computer.
Must be able to lift up to 15 pounds at times when necessary.
Fine motor hand movements typical of performing standard Complex Care Case Manager (RN) - Addiction duties.
