Summary

The Nursing Assistant performs complex age specific nursing care for adult and geriatric patients with a variety of physical, mental, behavioral or psychological needs. The incumbent continues to provide care under supervision of a Registered Nurse, and are learning to perform complex nursing skills, and will refer issues/ questions to supervisors, higher graded NAs, licensed nursing staff or other healthcare professionals.



Duties

The Nurse Assistant is accountable for adhering to established policies, procedures, standards of care and standards of practice and for maintaining competencies consistence with role and responsibilities. The distinguishing factor is that patient/ resident assignments typically involve more complex nursing needs which can vary within a range of predictable to unpredictable requirements. The Nurse Assistant may perform these duties in a variety of health care settings; outpatient, inpatient, community living care, acute and sub-acute. The Nurse Assistant will care for patients/ residents with acute to chronic conditions.

  • Utilizes glucose monitoring machine to receive glucose readings for patients, reports deviation to appropriate health care staff.
  • Provides direct care for complex medical and psychiatric conditions.
  • Makes observations of subtle changes in condition, i.e. behavior shifts, mood, skin coloring, appetite, etc. and takes action accordingly within the NA's assigned role.
  • Documents care given, observations and responses on appropriate tools or forms.
  • Responds appropriately to subtle patient changes by contacting the appropriate nursing personnel. Monitor and documents patient intake and output reports deviations timely and accurately.
  • Performs nursing procedures within scope of practice and according to policies and standard procedures to care for the Veteran for such procedures but not limited to: enemas, catheterization, colostomy care, skin care simple dressing, application of braces and prosthetic devices.
  • Works on small group activities with patients according to the patients treatment plan. Recognizes the residents' autonomy and promotes independence by means of respect, limit setting and individualized treatment.
  • Utilizes equipment to promote Veteran self-care, such as lift equipment and Patient Wandering System.
  • Verbally provides patient information to the treatment team for inclusion in the patients' plan of care, participates in treatment team meetings for resident care planning.
  • Enters information about the residents' condition in the chart, utilizing appropriate formats. Participates in maintaining a clean, safe environment for the patient/resident.
  • Removes defective equipment, reports hazards, observes safety regulations, contributes to infection control, and utilizes proper aseptic technique in performance of duties.
  • May serve as the unit safe patient handling liaison, or preceptor and mentor to other NA's. Orients members of the health care team to the units as appropriate or may function as a member of the unit based council.
  • Manages disruptive patient behavior by appropriately intervening and requesting assistance from other health care members as needed. Appropriately initiates basic life support, takes appropriate action.
Work Schedule: shifts varies including nights, weekends and holidays.
