Description:
Reads, understands and complies with all Standards of Conduct, JCAHO policies, CMS Pace Program Regulations and Standards, applicable State regulations and guidelines, PACE policies and procedures and all laws related to position responsibilities and employment at NeighborHealth;Upholds procedures and systems to safeguard the confidentiality of all patient and employee information. Upholds safety policies, practices, and procedures including safety, fire safety, electrical safety, proper body mechanics and material handling, office/ergonomic safety and other employee safety measures.This is a per diem, temporary position in the Transportation Department of Neighborhood PACE. Eligible start date 7/1/24 – 8/9/24. Hours may be flexible.The role involves escorting PACE participants from PACE vans to specialty offices at designated appointment times, providing guidance and support to participants while awaiting return rides, and coordinating a return to drivers/vans upon van arrival to hospital. The Transportation Assistant will communicate regularly with the Transportation Dispatchers to coordinate efficient participant transfers.Offers support and guidance to frail elderly patients attending specialty medical appointment and awaiting transport to and from appointments to/from home.Escorts/transports participants to/from PACE Vans to/from hospital specialty appointments – warm handoff to/from PACE Transportation Associate (Drivers) from curbside.Maintains communication with Transportation Dispatchers, coordinating transfers of participants to/from vans and specialty offices.Communicates participant concerns timely to Transportation office.Maintain excellent punctuality and attendance.Promote a sense of "team work" through demonstration of self-direction and self-motivation. Solve problems independently or knows when to seek consultation.Displays outstanding customer service skills when interacting with all PACE customers. Interacts with participants in a professional and respectful manner, which reflects the needs and concerns of the individual. Maintains a positive attitude. Uses communication devices appropriately. Adheres to all NeighborHealth and departmental polices and procedures.Participate in required departmental activities and meetings.Perform other related duties.Demonstrates commitment to PACE mission by actively promoting the autonomy and dignity of PACE program participants.Demonstrates the ability to communicate effectively and respectfully. Documents in accordance with protocol.Demonstrates knowledge of Participant Rights byactively protecting rights, including right to privacy;always treating participants with respect;encouraging and assisting participants in filing of complaints and grievanceshelping to maintain and clean and safe environment;
