Summary

The James J. Peters VA Medical center is currently recruiting for ONE Pharmacy Supervisor for the inpatient section of the Pharmacy Department.

The Inpatient Pharmacist supervisor must be a licensed pharmacist who is responsible to manage and coordinate comprehensive pharmaceutical care activities in inpatient care settings for medical and surgical programs.

Operates on a broad delegation of authority with independence of action to coordinate all aspects of pharmacy services during the day tour.

Duties

Some duties include but are not limited to:

Administrative/Supervision:


  • Manages the pharmacy inventory, drug accountability and controlled substances program for the Pharmacy during the day tour. Coordinates with other departments and agencies to effectively maintain resource accountability. Works in tandem with the inventory manager to ensure that all patients receive medications that are needed to treat their diseases. Responsibilities inclusive of support of overall Pharmacy budgetary savings plan, ensuring that the department-manages the conversions; patient education involving P&T decisions regarding cost saving initiatives.
  • Develops performance standards that are realistic, challenging and support the organizational mission, vision, and goals. Ensures that employees meet performance standards and competencies and identifies areas for improvement as needed. Effectively manages personnel and works towards resolving all labor. relations issues. Ensures that both professional and technical staff are adequately trained to meet the demands of the program.
  • Establishes work schedules and allocates staff as needed to provide adequate coverage. Maintains time and attendance records and approves leave. Will work with other supervisory staff to ensure that personnel are cross-trained and available to supplement areas other than their own. Keeps the management team abreast of issues regarding staff competence and performance.
  • Works with the Inpatient Pharmacy Program Manager to ensure continuity of patient care for patients being discharged during the day tour. Works with Chief of Pharmacy and Clinical Coordinator to ensure that pharmacy interventions are conducted during the tour. Develops and establishes policies and procedures defining operational requirements.
  • Effectively utilizes the Veterans Integrated Systems Technology Architecture (VISTA), Computerized Patient Record System (CPRS) and Bar Code Medication Administration (BCMA) computer systems and other automated pharmacy equipment and computer software systems.
  • Assists in assuring that the Pharmacy program is compliant with all regulatory and accrediting body requirements, i.e., JCAHO, FDA, ASHP, etc.

Program Management:


  • Functions as the Overall Pharmacy Supervisor during the day tour regarding planning, directing, controlling, and coordinating comprehensive distributive and clinical pharmacy services to the Medical Center.
  • Maintains good workflow patterns to keep inpatient prescription processing consistent and ensure that missing doses are avoided and addressed in a timely manner.
  • Effectively organizes workload, directs activities, plans short- and long-range objectives/goals, solves problems, and achieves established goals within budget and staffing constraints.
  • Works with other pharmacy managers and staff to identify areas of duplication of efforts and develops solutions to streamline processes, reduce waste, save costs, and improve patient care.

Clinical

  • The Clinical pharmacist provides appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers as appropriate.
  • Provides patient-specific therapeutic drug monitoring and communicates relevant findings and/or recommendations to other health care providers in charge of the patient both orally and in writing.

Monitoring will include prospective review and intervention in:
Therapeutic appropriateness of a patient's drug regimen. Therapeutic duplication in the patient's drug regimen. Appropriateness of the route and method of administration. Degree of patient compliance with the prescribed drug regimen. Drug-drug, drug-food, drug-laboratory, or drug disease interactions.

  • Completes medication histories including patient interviews.
  • Serves as a drug information resource by providing up-to-date drug information to prescribers, other health care professionals, patients, and caregivers.
  • Participates in patient care rounds, review charts, evaluates pertinent laboratory data, drug-drug, and drug-nutrient interactions, monitors for adverse drug effects, and screens for allergies.

Dispensing

  • Reviews all medication orders for appropriateness, drug selection, dosage, route of administration and the amount, prior to dispensing.
  • Assures that all prescriptions and actions meet proper eligibility criteria on a patient specific basis and that the National Formulary and Non-Formulary procedures are followed. This includes adherence to all national, VISN and local criteria or use treatment algorithms.
