Summary

These Licensed Practical Nurse positions are located at the Togus VA Medical Center for Primary Care, Home-Based Primary Care and Outpatient Specialty Clinics. The Community Based Outpatient Clinic openings are located in Maine (Lincoln, Calais, Lewiston, Togus, Caribou, Portland). These positions are full-time at 40 hours per week. This announcement is intended to fill current and future vacancies.



Duties

THIS OPEN & CONTINUOUS ANNOUNCEMENT (OCA) ESTABLISHES A STANDING REGISTER OF ELIGIBLE APPLICANTS, which closes on March 12, 2024. The initial cut-off date for referral of eligible applications will be December 26, 2023 with subsequent reviews every other week as additional vacancies occur, or on an as-needed basis, until positions are filled. The LPN has a broad work experience with demonstrated skills sufficient to resolve a full range of nursing and patient care problems. As a member of the nursing care team LPNs assist licensed nursing staff in the care of patients receiving outpatient care or home care who are in acute, sub-acute or chronic states of illness. Assignments may involve complex nursing needs which can vary within a range of predictable to unpredictable requirements.

Based on assignment and grade, duties may include but are not limited to:

- Responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life.

- Demonstrates knowledge of basic health care provider skills.

- Follows standard precautions and other infection control protocols as needed.

- Understands Veteran's rights and responsibilities, provides comprehensive care and motivating Veterans towards optimal level of functioning.

- Accurately records care delivery and observations using established oral, written and computer communications according to documentation guidelines.

- Provide a range of patient care and contribute to the completion of the nurse care plan and recovery of patients/residents.

- Observe patients physical or emotional status and refer deviations and/or problems to supervisor .

- Use of equipment, materials and supplies in simple diagnostic and treatment procedures such as bladder scan, continuous passive motion device, blood glucose monitoring machine, specimen collection, etc.

- Recognize and react to emergent patient care situations and intervene while waiting for assistance, for example, recognizing need for basic life support, controlling bleeding and assisting with behavior crisis, etc.

Work Schedule: Monday-Friday Various Schedules between 7:00am to 4:30pm; to be discussed at interview and determined on selection.
