DUTIES AND RESPONSIBILITIES:

1. Facilitate patient placement at time of discharge in collaboration with the counselor's recommendation. Sensitivity to reciprocal referral relations will be part of the planning process.
2. Meet with each patient individually (caseload of 30) to discuss and facilitate aftercare appointments.
3. Complete and document follow-up compliance calls made to the next level of care with the patient in the prescribed time frame.
4. Display an accurate understanding of the progression of a patient's treatment process and how that contributes to continuing care planning.
5. Serve as "patient advocate" to enhance a collaborative relationship to maximize the patient's, families, and referral's ability to make informed decisions regarding aftercare as it relates financially/clinically.
6. Communicate with multidisciplinary team (billing, medical, resource specialist and clinical staff) at regular intervals throughout the course of treatment. Will develop an effective working relationship to maintain appropriate cost and desired outcomes.
7. Manage all referrals and transfers within Caron Continuum and schedule appropriate appointments and transfer consults providing expert advice to senior management with clinical, financial, and family involvement issues.
8. Collaborate with the treatment team in development and execution of appropriate care based on ASAM criteria.
9. Complete releases for aftercare providers and all necessary documentation in Aura.
10. Work in conjunction with RRD's for continuing care requests needed for substance use disorders.
11. Remain informed of services provided in regional offices and collaborate with regional team on appropriate referrals.
12. Facilitate transportation and miscellaneous needs of patients and get them safely to next level of care, home, residence, etc.
13. Uphold confidentiality and professionalism at all times.

2. Must be able to walk and actively participate in activities throughout campus
3. Ability to sit for extended periods of time

EDUCATION / EXPERIENCE QUALIFICATIONS:

1. Bachelor's Degree in human services, social work or related degree preferred.
2. Minimum of 2 years of case management experience preferred.
3. 2 years continuous sobriety preferred, if in recovery.

KNOWLEDGE, SKILLS AND ABILITIES:

1. Uphold confidentiality and professionalism at all times
2. Excellent verbal and written communication skills required
3. Working knowledge of Microsoft Office products
4. Must demonstrate skills in achieving internal and external customer satisfaction
5. Will display an accurate understanding of the Treatment Planning Process, documentation and confidentiality regulations
