Summary

This position is for the Chief of Pharmacy Services at the Hopi Health Care Center located in Polacca, Arizona. Incumbent is responsible for all matters pertaining to the Pharmaceutical Services provided, including traditional drug distribution activities as well as extensive clinical pharmacy services. This position reports to the Professional Services Director.



Duties
  • Supervisory Functions
    • Formulates and implements overall pharmacy program plans to satisfy patient load requirements, priorities for services, and other health care objectives. Determines staff duties and resource allocation, while responsible for making adjustments as appropriate for workload
    • Serves as a pharmacy consultant and advisor on drug affects and on the selection of industry recommended drug therapy for individual patients. Promotes compliance with prescribed drug regimens and appropriate use of medication by the patient in the home through group or community-patient education programs and individual patient education using private consultation room for outpatients. ???
    • Performs a variety of functions and holds responsibility for the following areas: Budget Management; Supply/Acquisition Management; Pharmacy Contracts; Performance Improvement/Quality Assurance.
  • Non-Supervisory Functions
    • Function as a primary care specialist using established standards of care to provide primary care to patients with stable chronic conditions who require medication and to patients with acute self-limiting illnesses. Through dispensing, distributing and preparing medication, the incumbent assures pharmaceutical products are available and accessible that conforms to standards of quality for potency, purity and efficacy at the time administered or used by the patient. Inpatient
    • Pharmacy Services - Performs as an inpatient pharmacist and supervises all inpatient clinical activity and drug distribution, IV admixture services with responsibilities for planning training personnel and evaluation of quality of services utilizing national standards. Incumbent is expected to be on a 24- hour "on call" pharmacy service rotation and be available by telephone or other paging system, perform pharmacy duties on weekends, participate in emergency response code situations, and obtain and maintain BLS and ACLS training.     • Outpatient Pharmacy Services - Incumbent is responsible for assisting with the management of all pharmaceutical care programs, all prescription distribution functions, clinical and non-clinical program development, and clinical and community relations.
    • Clinical Pharmacy Services - Activities encompass all areas of pharmaceutical care and include (1) ensuring appropriateness of drug therapy; (2) pharmacotherapy consultation with medical and professional staff on drug therapy problems; (3) clinical pharmacokinetic services; (4) assisting in the development of patient care protocols; (5) develops, in conjunction with the medical staff, criteria for evaluation of medication use including, but not limited to, monitoring of therapy, drug utilization review, and clinical outcomes evaluation; (6) review of non-formulary medication requests prior to presentation to the Pharmacy & Therapeutics Committee for approval or denial. Helps to plan, design, and implement pharmaceutical primary care programs (e.g. anticoagulation, asthma, diabetes, hypertension, lipids, immunizations, and Patient Centered Medical Home).
