Statement of Purpose : To ensure adequate social service and elder resource-related services are available and provided to independent living residents, their families, and applicable resident representatives.awarded a top 10 spot on the national Fortune Best Workplaces for Aging Services 2020 and 2021 list, andMeets with all new Independent Living residents upon move-in to establish a baseline for support to identify areas of need such as powers of attorney, advance directives, and medical insurance.Facilitate and coordinate educational events related to relevant resident resources such as health plan open enrollment, long-term care planning, or advanced directives.Develop partnerships through community outreach and maintain an updated list of reputable elder resource providers in the greater community.Partners with the Resident Services Director to follow residents in the hospital or those transitioning to the HRC to anticipate needs or provide applicable resources.Identifies changes in affect, behavior, or personality such as depression, anxiety, and withdrawal, and reports these to the resident services team.Acts as the community liaison by communicating with and providing insurance providers with documents to ensure payment for covered medical services, such as private duty care, home health, veteran services, or long-term care policy execution.
