Scope
Understands needs and expectations of the patient in relation to services offered.Inform prospective resident and family of admission requirements, options and alternatives.Develop relationships with all prospective patients and their families.Obtains necessary information, completes and documents thorough assessments upon admission abiding by Medicare regulations, patient care and documentation.

Education and Experience Required
Must be licensed as a Registered Nurse in the state of Minnesota. Must obtain CPR certification within 6 months from date of hire.Must have a valid driver's license and use of an insured personal vehicle.Maintain his or her own competency with continuing education. Must complete and pass a state required background study.Able to physically perform the duties required by the position.Required to have the ability to read, write and speak English well enough to communicate with staff, residents and visitors.
Key Responsibilities
Build, manage and maintain patient and referral relationships.Assess prospective patient with goal of admitting patient same day as informational.Performs an average of two admissions per day in an eight-hour day.Assists with patient visits, as necessary. Perform thorough assessment and document per Medicare regulation in EMR.Report pertinent assessment information to all appropriate members of the interdisciplinary team after admission.Willingness to learn and successfully implement and execute the Knute Nelson Sales Training.Obtain necessary information upon assessment and admission of patient. When Social Services is not present, admission nurse to sign required admission paperwork with family.The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.
