SUMMARY OF JOB: A Triage RN receives and prioritizes all incoming and outgoing calls in order to continue delivery of safe, holistic care to a population with multiple co-morbidities and advanced chronic illnesses. The Triage RN implements the nursing process during phone encounters to assess the patient and caregiver situations and provide appropriate support including education of the disease process to the patient and family.

RECRUITMENT REQUIREMENTS: Minimum of 2-3 year med-surg experience required; 1 year of home care experience preferred; BSN preferred; ER/Triage experience a plus; able to work weekend, holidays; able to demonstrate professional competencies in a home care setting; able to efficiently utilize technology (cell phone, computer).



LICENSE AND REGULATORY REQUIREMENTS: Current State RN License, Current CPR Certification
