We are excited to present an opportunity to work as a Health Coach in the Department of Family Medicine and Community Health at our prestigious institution. As a Health Coach, you will collaborate with patients to achieve their health and wellness goals in a meaningful way. Through a supportive and evidence-based approach, you will assist individuals in making sustainable behavior changes, promoting autonomy and self-discovery. This position involves working individually or in small groups through virtual, telephonic, and face-to-face interactions.
Key Responsibilities:
- Build trust and rapport through attentive listening and effective communication.
- Create a safe and strengths-focused environment based on values.
- Foster patient autonomy and responsibility.
- Utilize evidence-based strategies to help patients explore their health goals.
- Assess patient readiness and commitment to change.
- Identify and support health improvement objectives.
- Align patient values with long-term goals.
- Assist in tracking progress, ensuring accountability, and addressing potential obstacles.
- Empower clients to develop effective behavior change techniques for sustainable success.
Requirements:
- Bachelor's Degree.
- Completion of Health Coach training approved by NBHWC.
- Board certification from NBHWC.
- Minimum of 2 years of direct patient experience.
