Practice Details:

Allina Health Home and Community Services is seeking b/c or b/e Geriatrician, Family Medicine or Internal Medicine Physician to join the Senior Health Co-Care Team.

The complex care team is a robust multi-disciplinary team which includes:

PhysicianFull-time Nurse PractitionerNurse triage 24/7Advanced Care Team: RN care coordinator, social worker, pharmacist, and care guide. About the practice: You will be part of a valued program within Allina Health that provides easy access for seniors to receive primary care in their home.In this role you will see patients in the North Metro area of the Twin cities with a patient panel of 100 patients.Excellian, our EMR, is a software product from Epic and is considered one of the most comprehensive systems in the nation.Qualifications:Active, unrestricted State of MN Medical LicenseNPI and DEA requiredMust be BE/BCBenefits:Up to $4500 annual CME allowance, plus additional $850 technology allowanceGenerous retirement savings options, including a 5% annual contribution and 401k matchMalpractice insurance coverage, including tail coverage at no costWhy Allina?
