The outpatient mental health nurse is a registered nurse responsible for coordination of care for veterans who are seen within the Mental Health Clinic.

The RN will provide care coordination to an assigned population, in collaboration with and as a member of the Behavioral Health Interdisciplinary Program (BHIP) team as well as provide coverage to the other mental health areas as needed.

The RN will demonstrate a therapeutic relationship with the acute and chronic mentally ill patient by focusing attention on the emotional and behavioral concerns through a mutually defined professional relationship.

The RN performs structured clinical assessments and complex, systematic psychiatric assessments which include the total health status of the patient.

This requires knowledge of medical terminology, diagnostic categories, and ability to perform chart reviews.

The incumbent begins the coordination process at the time the patient is identified and works collaboratively with staff within the Mental Health Clinics, as well as staff throughout the Veterans Health Administration (VHA) facility that may also be involved in his/her care.

The RN reviews assessments completed by others, and obtains missing information from providers, patients and medical records. The RN will demonstrate leadership in utilizing verbal and behavioral treatment modalities.

The RN will initiate patient and family education to include information about mental illness, medication, benefits and consequences of behaviors, coping strategies, evaluations and assessments and legal treatment modalities.

This includes the development of an appropriate treatment plan and/or safety plan for patients, and the ability to communicate that plan to the patient's provider.

The RN will provide for self-expression and processing of feelings in a group setting.

The RN will demonstrates extensive oral and written communication skills:

contacts patients through written correspondence and telephone calls, and conducts follow-up interviews and creates and maintains forms, surveys, instruments, and materials for proper coordination of the different treatment modules available.

The RN will demonstrate appropriate use of a computer. The RN must be willing to be placed, detailed, or temporarily assigned to other BHIP and/or locations, if necessary.
