Responsibilities
Provides psycho-social assessments, counseling and referral to community services and resources. Educates patients, families, and Woodland Healthcare staff. The Social Work department is a self- managed work team. The social workers are on call weekends and holidays.
- Provide psycho-social assessments for inpatient and outpatient units of Woodland Healthcare.
- Counsels patient and families for psycho social-related issues.
- Refers patient and families to community services and resources.
- Acts as a resource for education and community services for patients, families, and Woodland Healthcare staff.
- Participates in Quality Control and Process Improvement activities.
- Participates in interdisciplinary rounds and special conferences. Services on Woodland Healthcare and community committees.
Qualifications
Experience:
- A minimum of one (1) year of acute care hospital experience of a combination of hospital and other clinical experience
Education:
- Master's Degree - This position requires a Masters degree in Social Work.
Pay Range
$ $53.60 /hour
