The duties of the position include but are not limited to the following:


Housing and Urban Development - Veterans Affairs Supportive Housing (HUD/VASH) Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches in providing complex patient management.

Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings.

Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs.

This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems.

Coordinates care with multiple providers across all levels and sites of care.

The HUD/VASH RN develops a plan of care using information from the patient, family, and provider. Bridges the gap between discharge and primary care follow-up. Identifies high-risk patient population and transitions to appropriate discipline (i.e., Primary Care, Home Based Primary Care, Hospice/Palliative Care etc.). Responsible for intensive monitoring, follow-up, and clinical management of high-risk patients. Collaborates with interdisciplinary team members to develop and implement the Veteran Housing Stability Plan.

Contributes to the team-based assessment of the ability of individual Veterans to live independently in the community, as part of eligibility determination for Veterans with complex medical and psychological risk factors.

Provide recovery-oriented and Housing First services, with the goal of establishing the Veteran independently in the community, at the Veteran's highest level of functioning.


The HUD/VASH RN addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practice.

Evaluates care and outcomes to ensure timely and appropriate provision of services. Integrates knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex situations.

The HUD/VASH RN collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. Demonstrates performance and leadership that is broad enough to improve the care for a group of patients. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals. Evaluates HUD/VASH outcomes of evidence-based decisions and practice changes for individuals, groups, and populations. Responsible for the documented outcomes at the program or service level.

Program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility.


Work Schedule:
Monday through Friday, 8:00 am to 4:30 pm.
