Provides quality diabetes self-management education and medical nutrition therapy (limited to Registered Dietitians) in individual and group settings based on assessed needs.

Advocates for and communicates about improved quality of care and outcomes for those living with, at risk for, and affected by diabetes and cardio-metabolic conditions.

Track community diabetic health status, evaluate health trends, and risk factors of diabetic population groups. Collect data in a systemic and organized manner and analyze data. Collaborate with community leaders to evaluate the data, determine opportunities and needs for
the future.

Provide diabetes education and program services in orientation for new personnel related health workers, personnel on training programs and special projects personnel.

Plan and implement training programs for trainees, co-steps and summer students.

Certified Diabetes Care and Education Specialist or BC-ADM credential preferred; if not already certified upon hire, employee will obtain certification within two years of appointment.
