PSH Case Manager


We are hiring a PSH Case Manager to join our growing non-profit NWPP is a fun and supportive organization that provides meaningful impact within the Portland community.


Salary:
$56,600 Annually

Job Duties:

  • Maintain a caseload of approximately 15 very lowincome seniors who have experienced chronic homelessness.
  • Work collaboratively and respectfully with clients to develop effective housing plans to address immediate and longterm housing and healthcare needs.
  • Meet regularly with clients, primarily in communitybased settings and always with a strengthsbased and clientcentered approach, to assist with access to resources and services that support their housing and health stability.
  • Provide assistance with housing applications, advocacy with landlords, moveins into permanent housing, and/or eviction prevention services.

Job Qualifications:

  • Minimum of one year experience providing social services to adults from diverse backgrounds and cultures.
  • Interest in working directly with people who may be experiencing a variety of challenges, including poverty, aging, homelessness, and significant health issues.
  • Willingness to engage in meaningful training, selfexamination, and action directed at promoting diversity, equity, and inclusion in the organization.
  • Excellent interpersonal skills.
  • Experience working with computers, databases, and commonly used software.

Preferred Qualifications:

  • Demonstrated understanding of the root causes of homelessness and unique challenges faced by seniors of color and other historically marginalized communities
  • Demonstrated ability to provide culturally responsive services to vulnerable populations
  • Bachelor's degree from an accredited college or university is preferred
