Responsible for providing outreach services at the VA Salt Lake City Health Care System (VASLCHCS) Homeless and Justice Clinical Recovery (HJCR) Program with primary emphasis on assessment of Veteran needs, care coordination, facilitation of groups, individual psychotherapy, and provision of psychoeducational classes of Veterans who are homeless or at risk of homelessness.

Will also act as a resource consultant to members of both the VA interdisciplinary staff and community partners from across the catchment area.

Coordinates community resource referrals and community-based services, including information and referral for additional services from other VA programs, other government programs, and community agency programs.

Establishes and maintains effective therapeutic relationships with homeless Veterans and, as appropriate, their families.
Assists Veterans who have been referred to non-case managed HUD vouchers in the application process.

Assists in providing support and resources to Veterans who do not qualify for homeless program services due to being over income.

Coordinates, manages, and responds to requests coming from the National Homeless Hotline, consults, phone calls/messages, and community referrals, as well as coordinating follow-up for care provision and closeout.

Assists in coordinating a walk-in clinic for homeless Veteran's on VA campus that includes participation of other VA programs and community partners.

Assists with and participate in community events including the annual Homeless Veterans Stand Down, Point-In-Time Count (or PIT Count), and other events and activities directly related to outreach to Veterans who are literally and/or chronically homeless.

Provides outreach and crisis intervention services to homeless Veterans in the community, seeking to address the cause as well as the presenting complaint of homelessness.

Possesses the knowledge and ability to independently implement treatment modalities, provide counseling or psychotherapy for individuals, families, and groups, within the scope of licensure and training.

Develops treatment plans in collaboration with the Veteran/family/caregiver, with the interdisciplinary treatment team, and with appropriate community partners.

Works independently with Veterans and their families who are experiencing a variety of psychiatric, medical, and social problems utilizing individual, group, family counseling, and psychotherapy skills.

Independently provides consultation services to other staff regarding the psychosocial needs of homeless Veterans and the impact of psychosocial problems on health care and adherence to the treatment plan.

Participates in outreach efforts to encourage homeless Veterans to engage in care.

Assists in tracking data of outreach contacts by entering contacts in appropriate logs, databases, and programs (such as Microsoft Excel, the electronic health record, or HOMES).

When setting pay, a higher step rate of the appropriate grade may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade).


Paid Time Off:
