Seeking experienced social worker (Bachelor's in Social Work) with at least one year of work experience. Best qualified candidates will have a background in early childhood education, maternal child or department of children & families. Must be able to travel throughout Lee County area and comfortable going into patient homes. Bilingual is required fluent in either Spanish or Creole. This position will be mostly remote, but does have to come into the office for meetings and work as needed.

SummaryJOB SUMMARY: Responsible for the delivery of services to the Healthy Start client population as outlined in the Healthy Start guidelines. The Healthy Start Social Worker facilitates services across a continuum through collaboration with community resources, the Healthy Start team, and other professional disciplines to ensure effective and efficient delivery of Healthy Start services and the achievement of desired client outcomes. Supports the organization vision and mission. Uses knowledge of patients age and cultural diversity into the delivery of services rendered. Contributes to the success of the Healthy Start team through excellence in customer service and performance improvement techniques that demonstrate positive outcomes of said services. Teamwork, flexibility and professionalism, while important in all functions of the System, are central to the success of the Healthy Start Team, and are expected in this role. Duties will include participation in events sponsored by Healthy Start, as well as covering team member PTO, requested office tasks, and traveling outside normal work area regions to meet the needs of our client caseload and the needs of the organization.
Scope of Duties:
1. Counseling the client and family in meeting related needs.
2. Initiating appropriate preventive procedures
3. Coordination of services
4. Referral to other services as needed
5. Informing the physician and other staff of changes in the clients needs (Healthy start)
6. Participating in in-service programs
