Job Summary and QualificationsThe case management assistant (CMA) will work under the direction of the RN case manager or social worker to assist with facilitating patient discharges requiring post-acute placement or services, and will identify and report barriers to discharges. The CMA will provide administrative support in Case Management functions.
What you will do in this role:
- Assist with facilitating patient discharges.
- Identify and report barriers to discharges.
- Provide administrative support in the case management functions.
- Associates degree preferred
- Minimum of one-year experience in a health care setting required.
- Medical terminology required
- Healthcare professional (PCT, CNA, EMT) preferred

HCA Florida Blake Hospital is a 383 bed hospital. We are located in Bradenton, FL. We have a thirteen acre campus by the Gulf Coast beaches.
