Hospice Services of Lake County is seeking a per-diem RN Case Manager and a weekend on call nurse who will make a meaningful impact in the lives of our palliative and hospice patients receiving symptom management.
The successful candidates will collaborate with interdisciplinary team members to manage symptoms, provide education, and coordinate support for the patient and caregivers.
We are continuing to grow and expand our services offered to Lake County residents with a focus on offering a supportive team environment for employees and volunteers daily.
The Registered Nurse Case Manager plans, organizes, and coordinates care under the Medical Director's orders with other Interdisciplinary team members.
Care and family support are provided in the patient's place of residence whether it is a private home, a residential care facility, or a skilled nursing facility.
The weekend call nurse working from home or location of choice within a 45 minute drive or less to most Lake County locations will provide telephone or face - to -face support on weekends for patients and caregivers as needed.
*On Call Nurse - *Saturday 8:am - Monday 8:am. Special on call with regular wages for telephone calls and home visits. Overtime rules apply.
Current California RN license and a minimum of three years of experience preferred. Valid driver's license, reliable transportation, drug screening, and background check will be required.
